   It has been stated that Emergency Care has been a neglected topic in development of health systems, and that a major part of deaths in low- and middle-income countries are caused by conditions that could be addressed by a stronger focus on this. A Basic Emergency Care - Learning Lab adapted to the education and practical training needs in low-resource countries is being developed, with features that support training of important skills related to this field.

   Every year, there are 2.7 million newborn deaths and 300,000 maternal deaths - many of them in low-resource settings. There are educational programs and hands-on simulators that increase knowledge and skills of health care workers, but impact from implementation of these solutions can only be seen when there is adequate follow-up. This is currently very costly and therefore difficult to support. LIFT digital service is a suite of Learning Improvement and Facilitation Tools (LIFT) to support health care workers in low resource settings along their learning journeys. The goal of LIFT is to leverage digital technologies to reduce the burden of follow up by enabling increased accountability and quality of ongoing training and quality improvement efforts.

   With the proposed concepts we seek to aid challenges both within the health-area by strengthening the primary health care for woman and children, as well as within the education area by introducing new educational methodologies and tools supporting the mentors and students. The solutions will support innovative and proven educational methods like data-driven spaced repetition (low dose high frequency), simulation and adaptive eLearning, and help competency development of birth attendants. Capitalizing on digital opportunities for education and healthcare is a main strategy for Laerdal to reach our goals of helping save 1 million more lives per year by 2030. We will build on the Safer Births project (supported by Visjon 2030) and other digital solutions within Laerdal. We will further adapt and develop these technologies to support learning for a wider range of challenges related to competence development within maternity wards. Amongst others this can be normal labor management, management of complications in labor of management of severe bleedings. We are exploring three areas of innovation: 1) Standardized introduction modules that integrates videos and other digital formats to help a facilitator introduce new competencies with consistency and quality, as well as adapt to timeslots that are practical within different maternity wards. 2) A

   Every year, there are 2.7 million neonatal deaths, 300,000 maternal deaths, and 2.6 million stillbirths, the majority of which occur in low-resource settings and during the day of birth. Many of these deaths could be averted if attended by equipped, competent and confident health care providers. Cesarean section is one of the interventions that are known to prevent death and morbidity when women experience complications in childbirth. Timely cesarean sections can prevent 100,000 maternal deaths every year and reduce newborn deaths substantially too. Globally, there are currently two grave concerns. On one side, there is a massive underserved population where C-section is not available. WHO see a c-section rate around 10-15% as a natural proportion based on clinical indications. There is a massive need to train surgical teams to provide access for mothers and newborns. We will develop a reusable and comprehensive c-section simulator to support skill training and team training, covering the incision with realistic body layers, removal of baby as well as suturing and repair. The simulator will be a tool to introduce procedural skills for residents in training, support updates to competencies for surgical personell, a tool to discuss cases in situations of mentorship, as well as support team training of surgical teams to increase quality of performance. The simulator will be an important tool for implementation of the WHO safe surgery checklist.

   Around 140 million births occur every year in the world. The majority of these births have no identified risk factors at the onset of labor. Yet complications arise, and around 800 women and 7700 newborns die each day during pregnancy, childbirth and in the postnatal period. An additional 7300 women experience a stillbirth. A recent Lancet publication on patient safety (Kruk, 2018) shows that more people die of poor-quality treatment than access to treatment. Ensuring that comptency based traning and life saving equipments actually leads to improved care is of fundamental importance if we are to strengthen primary health care. It is very challenging to save the life of a newborn and a mother within severe emergencies such as birth asphyxia and severe bleedings. Research shows that preventing emergencies though improved essential care and improved management of critical situations has a very high life-saving potential. This project aims to improve quality of labor managment within three arenas of innovation: 1) An educational service to introduce and develop best practises for essential care for Labor and Birth 2) An educational service to introduce and develop best practises for managing complicated Labor and Birth 3) An educational service to introduce and develop best practises for Vacuum Assisted Delivery (VAD) 4) Solutions to support good decision making in teams during labor. We foresee innovative simulators to train assessment of progress of birth and management of fetal heartrate. We also foresee digital solutions for real time decision support during labor.
