FibroScan er et apparat som gjør oss i stand til å estimere leverstivhet på en standardisert og enkel måte. Det er et hjelpemiddel som det siste tiåret har blitt et integrert hjelpemiddel i en rekke utrednings- og behandlingsalgoritmer innen hepatologien. Både nasjonale, europeiske, amerikanske og asiatiske guidelines anbefaler bruk av elastografi i kombinasjon med biokjemiske markører for å identifisere pasienter som har høy risiko for alvorlig forløp av fettlever, for behandling av virushepatitter, og til å vurdere fibrosegrad som surrogat for sykdomsprogresjon i leversykdommer som for eksempel primær biliær cirrhose. Elastografiverdiene brukes i vurdering av igangsetting av kostbar behandling, evaluering av progresjon av sykdom, og for å gjøre en mer kvalifisert vurdering av hvilke pasienter som bør henvises til leverbiopsi, som er en invasiv undersøkelse med potensielt alvorlige komplikasjoner. Måling av fibrosegrad i lever kan gjøres ved transient elastography (TE -Fibroscan®) eller elastografi som eget program til ultralydapparat. Førstnevnte kan håndteres av sykepleier eller lege, og har validerte cutoff-verdier som brukes i tilgjengelige guidelines. Elastografi krever at en har et fullt ultralydapparat med software som muliggjør elastografi, og målingene må gjøres av person med ultralydkompetanse. Verdiene man får ut ved denne typen elastografi varierer fra produsent til produsent, og gir således ikke verdier som korrelerer direkte med verdiene som brukes i de fleste guidelines, blant annet de nasjonale retningslinjene for oppfølging og behandling av hepatitt C. For å ha nytte av målingene i daglig bruk på våre avdelinger, er det dermed transient elastography ved Fibroscan som er aktuell. Oppsummert må klinikken ha apparater som gir mulighet for elastografimålinger uten spesialkompetent personnell, og uten at de må investere i eget ultralydapparat. Dette er det for øyeblikket kun FibroScan fra Mediplast AS som gjør.
